Applying for a priest’s reference for Catholic primary school entry in September 2027

If your child was three years old on or before on 31 August 2026 and you wish them to attend a Catholic primary school from September 2027, please download and complete the reference form below.

• Download the priest’s reference form.
• When you have completed both sides of this form, please return it to the Parish Office (not to the school).
• Interviews will commence on Monday, 9 November and end on Friday, 27 November 2026.
• Please include original Baptismal Certificate if your child was not baptised at Our Lady of the Annunciation.
  Please ask the priest to return the original certificate to you at the end of your meeting.
• When you have returned the completed form to the Parish Office, please telephone 020 8654 1709 between
  9am–3pm only to arrange a time to speak with the Parish Priest.
• Please note the office is closed on Wednesdays and at weekends.
  Ideally, both parents should come, but if only one is available, it should be a Catholic parent.

• Regrettably, appointments will not be available after Friday, 27th November 2026
